Acute Residential Rehabilitation

 

This program operates in acute care hospital settings. The patient population includes residents requiring 24-hour nursing, residents in the early stages of the post-acute phase of recovery, and residents requiring evaluation for appropriate neurobehavioral treatment. Residents may be dependent in all activities of daily living. Treatment along the post-acute continuum of care may extend from short-term (30-180 days) treatment, to long-term care (greater than 180 days) for persons requiring care for ongoing medical/neurobehavioral interventions.

 

 

Community Re-Entry Program

This program is designed for those residents with potential for community re-entry. Each resident has an individualized program designed to increase independence of Activities of Daily Living (ADL) while participating in the vocational program or cognitive rehabilitation program for up to six hours per day. These residents may have long-standing mood, thought, seizure, cognitive, and/or behavioral disorders that are stabilized by ongoing neuropharmacological management, behavior management, and/or environmental structure.

 

 

Post-Acute Transitional Rehabilitation

This program is designed for those residents with potential for community re-entry. Each resident has an individualized program designed to increase independence of Activities of Daily Living (ADL) while participating in the vocational program or cognitive rehabilitation program for up to six hours per day. These residents may have long-standing mood, thought, seizure, cognitive, and/or behavioral disorders that are stabilized by ongoing neuropharmacological management, behavior management, and/or environmental structure.

 

 

Post-Acute Long Term Care

 

This program has the capacity to serve numerous individuals in a post-acute non-hospital residential setting. It is designed to assist individuals with severe functional, behavioral, and cognitive limitations requiring long-term or possible life care. It is designed to provide a stable and structured safe environment. Residents are supervised 24 hours a day and are actively involved in daily activities that are tailored to the needs of each person. Preserving a positive self-image and self-worth are important objectives integrated throughout the program.

 


Community Based Living

Our Community-Based Living Program provides an essential treatment component for the transition of brain injury survivors back into the community. Programs and services offered are designed to lead either to the maintenance of the maximum achievable independence level within a supervised setting or total independence within the community. Residents may enter the program as the final phase of Touchstone’s continuum of care or by transferring from another care setting.

 


Vocational Rehabilitation Program

 

It is widely acknowledged that individuals who have neurobehavioral difficulties often learn best by "doing." The vocational program offers such a forum in which a person has the opportunity to learn, practice, and integrate appropriate work behaviors with marketable skills. Targeted outcomes, which are individually determined, drive the vocational program for each participant.
